- Limited Personal Release Agreement (LPRA)
- Use this form for living individuals and family permissions. Please complete the form in black ink and keep it current. This agreement allows the Carpenter Family Project to use personal information, photographs, and documents for approved family history publication and research. Please update your LPRA at least once each year, and check whether an existing form is already on file. For inquiries, use the postal mailing address listed on this website. Please do not use email for this process.

- Family Group Sheet (FGS)
- Use this National Genealogical Society Family Group Sheet (FGS) when submitting documents with the LPRA. You may complete the form electronically before saving it to your computer. Use one form per family unless additional space is needed. Other family group sheets, such as those from FamilySearch, are also acceptable. If you have a subscription to Family Tree Maker, you may also use it to generate a person report by selecting the Publish tab and then Person Reports.
