Limited Personal Release Agreement (LPRA)
Please use a black pen when you fill out this form. For living individuals, I need your permission to grant and release material for publication to the Carpenter Family Project (CFP). This includes your personal photograph and documents, etc. Please update your LPRA at least once a year. Please ask whether you have an existing LPRA. For inquiries, use postal mailing contact address listed here at this website. Please do not use email.

Family Group Sheet (FGS)
Use this Family Group Sheet by National Genealogical Society (NGS) when submitting documents with the LPRA. You can fill out the NGS-FGS form before saving it to your computer. Use only one form for each family unless you might need more space.

There are other FGS similar to NGS like the Family Search FGS so any of these will suffice.

You may also use Family Tree Maker (2019 or 2024) if you have a subscription to Ancestry.com. To use the Family Tree Maker, select the Publish tab, and under Person Reports, select Individual Report.

The Family’s Inspirational Historian
Branches of the Heart is an adaptation from an old fable "The Little Red Hen”. It is a traditional folk tale, likely of Russian or English origin, passed down orally before being popularized in writing. It was first collected and published in America by Mary Mapes Dodge in 1874 for St. Nicholas Magazine. Common, well-known versions were later retold by Florence White Williams (1918) and Paul Galdone. Furthermore, the story was also a spin-off from the Adventures of Amberlon: Among the Oaks which was a recording created and registered at the Copyright Office in 2008 (PAu 3-470-655). Branches of the Heart was created specifically for CFP by Jesse Carpenter. All Rights Reserved.
